To Enter: Complete and follow the directions on the Registration Form available in Cincinnati Bell Yellow Pages Warren County Edition or send a 3x5 postcard with your name, address, and daytime phone number (with area code) to Cincinnati Bell Yellow Pages Best of Warren County Sweepstakes, PO Box 493, Mason, OH 45040. Lost, late, incomplete, illegible, mutilated, misdirected or postage-due entries are void. In case of a dispute, the authorized holder of the email account from which an entry is received will be deemed the entrant. All entries must be received by 11:59 p.m. EST on December 31, 2004. All entries become the sole property of Sponsor and will not be returned.

Prizes: Prizes: Five prizes will be awarded: (1) $1000 in gift certificates to participating Warren County businesses and (4) $500 in gift certificates to participating Warren County businesses. Prizes are not transferable and no substitution or cash equivalent is permitted. All applicable federal, state and local taxes are the sole responsibility of the winner. By entering, winners agree to be bound to any conditions imposed by the participating businesses on the use and redemption of the gift certificates. In the event that a prize becomes unavailable, substitutions of similar value may be made at the sole discretion of Sponsor.

Winner Selection: Winners will be randomly selected from all eligible entries. Odds of winning are based on the number of eligible entries received. Winners will be notified by mail and/or telephone. Winners will be responsible for providing identification and proof of residence and age. Winners may be required to sign an affidavit of eligibility. If any winner fails to do so, cannot be contacted, is ineligible or fails to claim a prize, the prize will be forfeited and Sponsor reserves the right to award the prize to a newly drawn winner. By accepting a prize, winner agrees to Sponsor’s use of winner’s name, voice, or likeness, without further compensation.

Limitation of Liability: Entrants release Sponsor and its officers, directors, employees and agents from all liability, damages or claims for any injury, damage or loss sustained in connection with any events that occur during, or as a result of the acceptance, receipt, use or ownership of any prize Sponsor is not responsible for, and shall have no liability with respect to any problems or technical malfunctions of any computer on-line systems, servers or providers, interrupted or unavailable network, server or other connections, miscommunications, computer or software malfunctions, telephone transmission problems or technical failures, computer equipment, software, failure of e-mail on account of technical problems or traffic congestion on the Internet or at any Web site or combination thereof, or other errors or malfunctions of any kind whether human, mechanical, electronic or otherwise. Sponsor reserves the right to disqualify any individual suspected of tampering with the entry process or the operation of the sweepstakes or website, or acting in violation of the Official Rules. Any attempt to deliberately damage the content or operation of this sweepstakes or the website is unlawful and subject to legal action.

All decisions of Sponsor are final and binding.
